This is where you get all the necessary paperwork for Accelerate services and events. It is now required that all students have a completed form on file before riding Accelerate vehicles or attending off-site events. Additional paperwork will be available as needed for overnight trips and events. Parents, please help us as we do our best to ensure quality and safety for your children.
This form must be completed before attending any event and/or riding any transportation provided by Accelerate and First Assembly of God. Right click and save the file, and bring to the next service attended. Thanks for your cooperation
